The Waitlist Application Form is available online as a Microsoft Word file, which you can print, fill out, and return to the Center. Or if you prefer, there are additional application forms at the Center's front desk. You MUST complete and return an application form to be placed on wait list. We offer a student rate reduction for low income families, and you may ask for more information about this rate at the time of application. The Diversity Survey is available as a Microsoft Word file, which you can print, fill out, and bring into the Center. There are additional Diversity Surveys at the Center's front desk. The Diversity Survey is optional, but we encourage you to fill it out. We
are proud of the diversity of children and families at the Center! We
would love to include your families' stories, games, foods, music, etc.
in our curriculum. Please help by returning this Survey. The Health Form is available as a Microsoft Word file, which you can print, fill out, and bring into the Center. There are additional Health Forms at the Center's front desk. The Health Form includes questions about your child's immunizations,
disease history, and medical needs. If you have any additional
medical concerns, please talk to your child's teacher or one of the Center
staff. This form must be signed by your child's physician. We
must have this form signed and on file within one week of your child's
enrollment in the Center. With this form, we also need a copy of
your child's immunization records. This is a requirement of the
Texas Department of Human Services, which licenses our Center. We
CANNOT make any exceptions! Please see our Immunizations
page for more information on Texas state requirements for immunization. We
encourage you to bring any questions or concerns to your child's teacher
or to Center staff. The Infant Supplemental Form is available as a Microsoft Word file, which you can print, fill out, and bring into the Center. There are additional Infant Supplemental Forms at the Center's front desk. The Infant Supplemental Form is a supplement to the Teacher Information Form. This form contains information about eating, napping, and other issues of concern to infant parents and teachers. It is a way to help the teachers get to know their infants. All parents of infants are asked to fill out an Infant Supplemental Form at the time of enrollment in an infant classroom. The Medicine Form is available as a Microsoft Word file, which you can print, fill out, and bring into the Center. There are additional Medicine Forms at the Center's front desk. The Medicine Form gives permission for the Center to administer prescription
and non-prescription medications. The Center staff are NOT allowed
to administer any medications without written permission. The form
must be accompanied by the medication IN THE ORIGINAL CONTAINER. The
Center staff is NOT allowed to administer any medications which do not
have the original labeling on the container. Please note the form
and medicine must be given to the staff at the front desk. PLEASE
DO NOT take any medicines to the classrooms, for safety reasons. The Permission Form is available as a Microsoft Word file, which you can print, fill out, and bring into the Center. There are additional Permission Forms at the Center's front desk. The Permission Form must be signed and returned within one week of your
child's enrollment. The Registration Card is available as a Microsoft Word file, which you can print, fill out, and bring into the Center. There are additional Registration Cards at the Center's front desk. The Registration Card contains all the general information needed to
enroll your child at the Children's Center. Some of the information
is required by the Texas Department of Human Services, which licenses
our facility. All of the information is essential to the well-being
and safety of your child. You must completely fill in all areas
of the card on both pages, sign, and return it to the Center at the time
of enrollment. The Teacher Information Forms are available as a Microsoft Word file, which you can print, fill out, and bring into the Center. There are additional Teacher Information Forms at the Center's front desk. Birdie Wing Teacher Information Form. Sea Wing Teacher Information Form. Butterflies & Bumblebees Teacher Information Form. The Teacher Information Form asks a number of detailed questions about
your child's background and interests. It is used by the teachers
to get to know their children. All parents are asked to fill out
a Teacher Information Form at the time of enrollment. In addition,
for children entering an infant or toddler classroom, an Infant or Toddler
Supplemental Form may also be required. The Toddler Supplemental Form is available as a Microsoft Word file, which you can print, fill out, and bring into the Center. There are additional Toddler Supplemental Forms at the Center's front desk. The Toddler Supplemental Form is a supplement to the Teacher Information Form. This form contains information about eating, sleeping, toileting, and other issues of concern to infant parents and teachers. It is a way to help the teachers get to know their toddlers. All parents of toddlers are asked to fill out an Infant Supplemental Form at the time of enrollment in a toddler classroom. |
